Welcome to our Reliability Instrument on spiritual formation and development. This specific instrument regards belonging and community, and will be combined with three other instruments to create the final Spiritual Formation Index. It will have the potential to be used by God to impact thousands of lives.
We appreciate your responses to each of the items. You will be asked to read a short descriptor and respond to whether the item applies to you, from a range of “Strongly Disagree” to “Strongly Agree.” Please resist from selecting too many “Neutral” responses as it decreases the usefulness of this process. Simply answer honestly and consistently about yourself.
All of your answers are entirely anonymous, so no one will ever know how you answer. As you (and others) respond honestly about yourself, you will provide us the data from which we can make this a much shorter and yet more helpful instrument.
